Medical Records Release Authorization

RECORD REQUESTS MUST BE RECEIVED BY 11/30/2023

To request medical records, submit the required Authorization for Release of Protected Health Information form and identification until 4:00 pm, Thursday, 11/30/2023. 

To request medical records: 

1) Download and complete the required Authorization for Release of Protected Health Information form; it Is also available at a drop box located outside St. Mark's front lobby.  Note: The completed form is required for processing your request. 

2) Add a scanned, readable copy of valid identification (e.g. State driver’s license or ID, U.S. passport, U.S. military ID, Resident alien ID or “green card”) and submit via:

Completed Records Requests will be MAILED via USPS to the address listed on your release of information form.
